Abstract :The purpose of this research was to clarify the current state of awareness regarding the individualized transition plan in Japanese special education by means of a survey questionnaire. In addition, it aimed to propose a concrete method for the effective use of the individualized transition plan at the ad val school level. The investigation was conducted among 50% of the Special education high school teachers nationwide, and a 66% response rate was obtained. As a result, many of teachers were found to have a high awareness of the individualized transition plan. Moreover, in accordance with their level of awareness, they had a proportionate affirmative opinion regarding the individualized transition plan. In future, it is expected that there will be much proactive development and use of the individualized transition plan in special education high school. When forming an ITP at the educational level, the results of the present investigation, and findings concerning the teacher's awareness ofHandling and management method of informationmay well be valuable points of reference..
